Welcome to the registration portal for the CAHMI’s Cycle of Engagement (COE).

Welcome to the CAHMI’s Cycle of Engagement (COE) Registration Portal.

Follow the 4-steps below to get a Cycle of Engagement (COE) account and personal dashboard. You will then be able to:

  1. Customize your Well Visit Planner® (WVP) family website and get your WVP Use Portal to access your WVP Data Dashboard, child and family data (e.g., Clinical Summaries and data files), and family engagement resources
  2. Customize your Online Promoting Healthy Development Survey (PHDS) family website and get your PHDS Use Portal to access aggregate quality reports and quality improvement resources

Overview of the Four Step Cycle of Engagement (COE) Registration Process

Step 1:

Decide who the primary account holder will be

  • Confirm the primary account holder (e.g., an individual, a clinic, or an organization)
  • Identify additional account holders to assist in managing customization settings and retrieving, reassigning, or sharing WVP data on the data dashboard
Step 2:

Create and verify your account

  • Confirm the COE tools you’re interested in using
  • Provide primary account holder’s email and create an account password
  • Verify the account via email from info@cycleofengagement.org to the primary account holder’s email
Step 3:

Complete your COE registration

  • Answer questions about the primary account holder, the practice or organization, and characteristics of families served
Step 4:

Get your COE Dashboard and customize your tools

  • Sign-in to your new COE account and dashboard
  • Complete the steps to customize your WVP and PHDS
  • Access resources to implement the WVP and/or PHDS with the children and families you serve

Preparing to Register and Customize

With a COE Account, You Can:

  • Access your COE Dashboard and manage the customization of the WVP and/or PHDS family websites
  • Access your WVP and PHDS Use Portals where you track use, get data reports, and family and implementation resources (e.g., Implementation Roadmap, Family Engagement Toolkit, Quick Guides to Get Results)
  1. Use your WVP Use Portal to:
    • Get your unique website link and QR code to share with families
    • Download personalized WVP family flyers to engage families
    • Send personalized WVP email or text invitations to families
    • Track use of the WVP by families
    • Access your WVP Data Dashboard to view and download At-a-Glance Clinical Summaries and Well Visit Guides to facilitate comprehensive, guideline-based Personalized Connected Encounters during well-child visits
    • Download a data file with child-level responses and results from the WVP questions and screeners. Use the data for quality improvement activities, reporting, research, and other purposes (NOTE: a data sharing agreement must be completed)
    • View and modify your customized WVP
    • Add or link to additional questions and/or assessments you want families to complete
    • Link to additional resources to share with families through their Well Visit Guides
    • Get implementation resources and support, including family flyers, office/visit room posters and invitational scripts, example implementation workflows, resources to engage other providers, FAQs and tips and troubleshooting resources
    • Get piloting and quality improvement resources
  1. Use your PHDS Use Portal to:
    • Get your unique website link and QR code to share with and invite families to use your customized PHDS
    • Track completions of the PHDS by families
    • Generate aggregate PHDS quality of care reports to help you track what is going well and improve the quality of well child services provided
    • Obtain professional credits, American Board of Pediatrics Maintenance of Certification credit or CME credit (coming soon!)
    • View and modify your customized PHDS
    • Add or update any additional resources you want to share with families in their PHDS Family Feedback Report
    • Get resources to implement the PHDS, including family handouts, email, text and verbal scripts to use to engage and invite families and example practice workflows, readiness checklists, FAQs and tips and troubleshooting resources
    • Get resources to use the PHDS in quality improvement or research projects

What to Expect When Customizing your Well Visit Planner (WVP) and/or Online Promoting Healthy Development Survey (PHDS)

Once you get your COE account and dashboard you can customize the WVP and/or PHDS to use with the children and families you serve. You will need to:

Create a simple WVP and/or PHDS unique URL/website link  to share with families
Add a logo/picture to personalize your WVP and/or PHDS family website(s)
Add links to any additional resources you want to share with families in their WVP family Well Visit Guides and/or PHDS Family Feedback Reports
Learn about and add additional assessments that go beyond the core WVP
