Welcome to the registration portal for the CAHMI’s Cycle of Engagement (COE).

Welcome to the CAHMI’s Cycle of Engagement (COE) Registration!

This COE registration portal enables you to get a Cycle of Engagement (COE) account and personal dashboard where you can:

  1. Create a customized Well Visit Planner® (WVP) family website, get your WVP Use Portal and Data Dashboard to access WVP Clinical Summaries and other resources
  2. Create a customized Online Promoting Healthy Development Survey (PHDS) family website and get your PHDS Use Portal, aggregate quality report generator and resources

Overview of the Four Step Cycle of Engagement (COE) Registration Process

Step 1:

Decide who will be the primary account holder

  • Confirm the primary account holder (e.g., an individual, clinic or an organization)
  • Identify others who should have account privileges to manage your account and Data Dashboard and sharing of data with other COE account holders
Step 2:

Create and verify your account

  • Confirm the COE tools you’re interested in
  • Provide primary account holder’s email and create an account password
  • Primary account holder responds to an email from info@cycleofengagement.org to verify their account
Step 3:

Complete your COE registration

  • Answer questions about the primary account holder, their practice or organization, and characteristics of families served
Step 4:

Get your COE Dashboard and prepare to customize the tools

Preparing to Register

With a COE Account You Can:

  • Access your COE Dashboard and manage the customization of the WVP and/or Online PHDS family websites
  • Access your WVP and PHDS Use Portals where you track use, get data reports and family and implementation resources (e.g., Implementation Roadmap, Family Engagement Toolkit, Quick Guides to Get Results)
  1. Use your WVP Use Portal to:
    • Get your unique website link and QR code and resources for engaging and inviting families to use the WVP
    • Track use of the WVP by families
    • Access your WVP Data Dashboard to view and/or download child/family Well Visit Guides and provider/user At-a-Glance Clinical Summaries for use and preparing for and conducting comprehensive, guidelines-based Personalized Connected Encounters with the children and families you serve
    • View and modify your customized WVP
    • Add additional questions and/or assessments you want to get child/family results on
    • Add additional resources you wish to share with families in the family Well Visit Guides
    • Get resources to implement the WVP, including family flyers, office/visit room posters and invitational scripts, example implementation workflows, resources to engage other providers, FAQs and tips and troubleshooting resources
    • Get resources to pilot and refine your plans to use the WVP in quality improvement projects or research and get feedback from families
  1. Use your PHDS Use Portal to:
    • Get your unique website link and QR code and resources to invite families to use your customized Online PHDS
    • Track completions of the Online PHDS by families
    • Generate and get aggregate Online PHDS quality of care reports to help you track what is going well and improve the quality of well child services you provide in partnership with the families you serve
    • View and modify your customized Online PHDS
    • Add or update any additional resources you want to share with families in their PHDS Family Feedback Report
    • Get resources to implement the Online PHDS, including family handouts, email, text and verbal scripts to use to engage and invite families and example practice workflows, readiness checklists, FAQs and tips and troubleshooting resources
    • Get resources to use the PHDS in quality improvement or research projects

What to Expect When Customizing your Well Visit Planner and/or Online PHDS

Once you get your COE account and dashboard you can customize the WVP and/or Online PHDS to use with the children and families you serve. You will:

Create a simple WVP and/or Online PHDS unique URL/website link name to share with the families you serve
Add a logo/picture to further customize your WVP and/or Online PHDS family website(s)
Add links to any additional resources you want families to be able to access in their WVP family Well Visit Guides and/or PHDS Family Feedback Reports
Choose among options to add additional assessments to the WVP that go beyond the Bright Futures Guidelines aligned assessments and content tailored for 15 visit ages

Create a COE account to begin using the Well Visit Planner® and/or
Online Promoting Healthy Development Survey to engage families and improve well care today!
