Why should I add additional account holders? A brief family-completed, online pre-visit planning tool where families share strengths and goals, complete recommended developmental, psychosocial screeners; pick priorities and get a Well Visit Guide and resources. Providers receive a Clinical Summary.
A post-visit family-completed survey that yields a set of validated quality indicators aligned with Bright Futures’ implementation standards and produces a feedback report to partner in improving quality. Providers receive an aggregate data report after at least 25 families respond.
A three-step whole child approach for families of children and youth with special health care needs to engage, plan, and improve care planning and outcomes in partnership with their child’s care team(s).

The Cycle of Engagement (“COE”) tools for early childhood use a family-centered, data-driven relationship-based approach to promoting whole child, equitable and integrated well-child care. The COE tools include the Well Visit Planner (“WVP”), the Online Promoting Healthy Development Survey (“Online PHDS”), and the CARE_PATH for Kids (“CPK”). Each tool has a family website and use portal. The use portals can be accessed by registering for an account through the COE website.
All COE websites were developed and are maintained by the Center for the Advancement of Innovative Health Practices operating as the Child and Adolescent Health Measurement Initiative (CAHMI). CAHMI created and makes the COE model and tools available for use by clinicians, practices, organizations and families to assess and improve the quality of child care and promote child and family health.
Cycle of Engagement Family Tools
The purpose of the COE tools is to empower and engage families to become partners in their child’s healthcare. The WVP allows parents/caregivers and families to explore key issues and needs important to the health of their child and family; learn about important topics to discuss with their healthcare provider; pick top priorities and receive educational information before, during and after visits; and partner with their child’s healthcare provider by sharing a personalized visit guide. The WVP is based on national recommendations and is made for parents/caregivers of children who are between 4 months and 6 years old. The Online PHDS is a parent/caregiver-completed survey that assesses whether young children (under the age of 6 years) are receiving nationally recommended preventive and developmental services based on the most recent, national AAP Bright Futures Guidelines.
Cycle of Engagement User Portals
Providers and care teams can access their WVP and Online PHDS Use Portals on the COE website. The user portals provide implementation and other resources for the WVP and Online PHDS. In the user portal, providers may customize their WVP and Online PHDS Family Websites; track basic usage of the tools by families; generate summary reports for quality of care provided; and get access to educational, implementation and family engagement resources. The Center for the Advancement of Innovative Health Practices shall retain exclusive right, title and interest (including all intellectual property rights) in and to all materials that are part of the website (including past, present and future versions), including, without limitation: art, icons, graphics, layout, text, images, audio and/or video, designs, features, advertisements, logos, domain names, trade names and marks, and service marks, any and all copyrightable material, the “look and feel”, the compilation, assembly and arrangement of the materials of the Service, and all other materials or content made available in the website and any such website content is protected from unauthorized use, copying and dissemination by applicable copyright, trademark, patent, publicity and other laws, rules, regulations and international treaties.
Well Visit Planner
Oregon Health & Science University (OHSU) owns and retains proprietary rights in the WVP. All content, formats, information, and functionalities of this site are exclusively licensed to and copyrighted by the Center for the Advancement of Innovative Health Practices DBA CAHMI.
Purpose of Use
We only use your personal data for specific purposes, as explained in the Privacy Notice for the COE website. Examples of how we use your information and data include:
You agree that Maryland law (regardless of conflicts of law principles) shall govern this Use Agreement, and that any dispute arising out of or relating to this Use Agreement shall be subject to the exclusive jurisdiction of the circuit court for Baltimore City, Maryland; provided, however, that if any dispute may be brought only in a federal forum, it shall be brought and conducted solely and exclusively within the United States District Court of Maryland.
Modifications to the website and the Services: We reserve the right, for any reason, in our sole discretion, to terminate, change, suspend or discontinue any aspect of the website, including, but not limited to, content, features or hours of availability. We may also impose limits on certain features of the website or restrict your access to part or all of the website without notice or penalty.
This Use Agreement governs your use of all Cycle of Engagement websites. The websites are made available to you by CAHMI. We may, from time to time, modify, amend, or supplement this Use Agreement. Such modifications or supplements shall be effective immediately upon posting in the website. You are responsible for periodically checking the website for changes to this Use Agreement. BY USING ANY COE WEBSITE, YOU ACCEPT AND AGREE TO THIS USE AGREEMENT AS APPLIED TO YOUR USE OF THE WEBSITE AND AS MAY BE AMENDED. If you choose to download or print your aggregate PHDS feedback report, any Visit Guides that may be stored on your WVP Use Portal, or an aggregate WVP report, CAHMI will not be liable for any actions related to your choice to disseminate, distribute, copy or rely upon the aggregate feedback report or print portions of an aggregate PHDS feedback report, any Visit Guides, or an aggregate WVP report with your name or any information contained in any such data.
In no event shall CAHMI or OHSU be liable for any incidental, special or consequential or indirect damages arising out of or relating to the use or inability to use WVP, PHDS, CPK, or COE websites.
Cycle of Engagement Provider and Family Accounts, Fees, Billing, and Termination
Without limiting any other remedies, we may suspend or terminate your Account and your access to and use of the website at any time if you are, or we suspect that you are, failing to comply with this Agreement with or without notice to you. We reserve the right to stop offering and/or supporting the website at any time for any valid commercial reason, at which time your license to access and use the website will automatically be terminated without further action.
For the convenience of our users, the website may provide certain links to other applications, services and websites provided by third parties. CAHMI is not responsible for the content of any other applications, services or websites linked to or from the website. If you follow any such links, you leave the website and you do so entirely at your own risk. CAHMI provides links from the website solely as a convenience to you and in no way should this be interpreted as a referral or endorsement of any content, sponsor or owner of any other applications, services and/or websites. This Cycle of Engagement website was developed and is maintained by the Center for the Advancement of Innovative Health Practices DBA Child and Adolescent Health Measurement Initiative (“CAHMI” or “we” or “us”). This Privacy Notice describes the information we process to support the Cycle of Engagement (“COE”) website, which includes the Well Visit Planner (“WVP”) and Online Promoting Healthy Development Survey (“PHDS”) Use Portals and CARE_PATH for Kids (“CPK”) website.

We will keep personal data for no longer than is necessary for the purposes for which it was collected and then we will securely delete or destroy it.
How your information is protected and secure
Data Security of Provider Data
The information gathered from you is confidential. The email address that you use for registering an account or provide within either the WVP or Online PHDS Use Portal will not be shared with any third parties, nor will your email address be sold or used for purposes other than for us to share educational resources or sending updates about the COE. If you no longer wish to receive resources or updates, you can unsubscribe at any time.
Data Security of Family Data
The information gathered about the users (e.g. caregivers/parents of children you serve) that you invite to use your COE Family Websites is confidential. We do not save any personal health information and we may share patient’s individual information with you as the Provider.
Providers who have an account with the COE and have enabled use of the WVP Family Website can request for completed Visit Guides with or without minimal identifiers to be stored on their WVP Use Portal. Depending on customization choices made through the WVP Use Portal and use of the WVP Family Website, we may collect and save two pieces of identifiable information, termed a child-specific special code. This information is encrypted and stored in a manner consistent with HIPAA and does not allow us to plausibly identify users. Details of information collected to generate the child-specific special code are below.
(a) date caregiver/parent completed the WVP on your WVP Family Website
(b) child’s first, middle (optional) and last name initials,
(c) month and year of child’s birth (not the day), and
(d) a special keyword selected by the caregiver/parent (like a favorite animal, food, toy or color).
Each data point is separately encrypted in CAHMI’s HIPAA compliant, secure WVP website database using highest standards of data security. Even if this information were accessed, it is implausible it could be used by anyone other than the provider and caregiver/parent to identify the child. For additional information on data security of family data collected from the COE Family Websites, please contact the CAHMI at firstname.lastname@example.org.
Providers who have an account with the COE and have enabled to use the Online PHDS Use Portal can generate summary reports of aggregated findings based on parents’ responses on the Online PHDS parent tool, specific to their practice or organization. Providers will not know what the parents’ individual responses are nor be able to identify who completed the Online PHDS.
Managing Your Personal Data
You are solely responsible for all activity on or related to your account, your customized content, and for the security of your computer, mobile device, and any other device that you use to access the website. You should not reveal your account password to other users or permit others to access your account. All acts performed using your account shall be deemed to be your acts. You shall promptly notify us of any changes to your account information or if such information has been disclosed to a third party in violation of this Privacy Notice. You shall solely be responsible for any harm or liability that arises from any improper third-party disclosure and/or use (including unauthorized third-party use) of your account. If there is a risk of your account being improperly used by a third party, you shall promptly take all reasonable measures to limit such use, including, without limitation by changing your password and notifying the CAHMI. Under no circumstances shall CAHMI be liable to you for any loss you incur, from your loss of any account information, unauthorized access or use of your account and/or from harmful or malicious code.
In no event shall CAHMI or any of its directors, officers, employees, agents, affiliates, or content or service providers be liable for any indirect, special, incidental, consequential, exemplary or punitive damages arising from or directly or indirectly related to the use of, or the inability to use, the website or the content, materials and functions related thereto, including, without limitation, loss of revenue, or anticipated profits or lost business or lost sales, even if such entity has been advised of the possibility of such damages.
